If you are unable to re-certify online, you may download the file in the Documents section and submit it in hard copy.
This form ensures that the certified MBE/WBE firms participating in Cook County contracts for credit are within the parameters of the Cook County Procurement Ordinance.
Please note that if you are currently certified with the City of Chicago in a non-construction area i.e., professional services or goods, the County Code requires: 1) the firm’s average annual gross receipts over the five most recent years does not exceed the U. Small Business Administration’s Size Standards and, 2) the personal net worth of the qualifying (51%) individual(s) does not exceed the County’s personal net worth limitations of approximately $2.2 million.
MBE/WBE/VBE Certification Program is valid for five (5) years.
On the fifth year anniversary date a Re-Certification Affidavit is required.
Provided that the agency’s requirements are met, you will no longer be required to submit your annual No Change Affidavit or Re Certification Affidavit to both agencies, just one.
For bids or proposals, professional services or non-construction firms certified with the City of Chicago, a MWBE Reciprocal Certification Affidavit must be submitted with the City’s letter of certification.
The Office of Contract Compliance will provide a Self-Service Kiosk for vendors to submit their online applications beginning January 8, 2019.
The Self-Service Kiosk will be available on Tuesdays and Thursdays from AM to PM and PM to PM by appointment only.
